What You Need to Know
- There is no official IRS or federal form labeled as “4022”
- Scammers are using this form to collect sensitive information including tax ID numbers, banking information, and other confidential data
- These requests may come through email, postal mail, or even phone calls

- Do NOT respond or provide any information to those requesting this form
